Simply visit our website at www.BirdsongSpeechTherapy.com, and click on "Contact Us". You'll find a link to our on-line referral form on the right. You'll also find a downloadable .pdf version if you prefer.
Many insurance carriers require physician referrals for speech therapy, so we recommend always submitting a referral (both to us and to the insurance carrier) to be on the safe side.
